Connect trail palm to lead thumb knuckle at setup

To load the trail arm properly, connect the trail palm to the lead thumb knuckle at address; once that alignment is set it automatically pins the trail arm underneath the lead arm. The commonly taught cue of having the palm toward the sky and the elbow pointing up is the effect of this connection, not the cause, so line the trail palm to the lead thumb knuckle first.
